Patient Choice - The Right to Choose[Return to Top of Page]

Increasingly, patients and family members are taking a more active role in decision-making about their health care and the Danbury VNA fully supports and encourages this practice.

Federal and state legislation guarantees a person’s right to choose the home health care provider that he or she wants in their home. This is an important right for health care consumers. Hospital discharge plans must include an evaluation of a patient’s possible need for medically necessary home health care. Hospitals are required to provide a list of agencies that offer service in the patient’s area of residence when home care needs are identified.

The Danbury VNA will provide services to patients who are discharged from any hospital in Connecticut, New York, New Jersey, Massachusetts, Rhode Island, Vermont, New Hampshire and Maine or any hospital in the country.
