Scope of Services
The Danbury VNA provides home health care
visits to patients of all ages regardless of ability to pay. We are the
area’s largest home health provider, having the expertise and capability
to serve patients in Danbury, Newtown, New Fairfield, Bethel, Southbury,
Woodbury, Redding, Ridgefield and Brookfield and other Northern Fairfield
County towns.

Hospital Liaison Services
Danbury VNA offers a variety of skilled
nursing and rehabilitation services to ass ist you during your
recovery period. Danbury VNA has been ranked as one of the top home care
agencies by Medicare.
If you or your loved one are referred to
Danbury VNA for home care services during their hospital stay,
our liaison at Danbury Hospital will assist you in transitioning from the
hospital to home. A definitive home care plan for home care services,
supplies and equipment will be coordinated in advance of discharge
making the transition from hospital to home worry free.

Remote Telehealth Monitoring
The
Danbury VNA, in collaboration with the Visiting Nurse Services of
Connecticut, is the Danbury area's exclusive provider of HomMed Health Monitoring System to
collect patients’ vital signs on a daily basis. With this system, the
Danbury VNA puts small, user friendly, computers into a patient’s home to
gather clinical data, regarding trends in their health status.

Increasingly, patients and family members are
taking a more active role in decision-making about their health care and
the Danbury VNA fully supports and encourages this practice.
Federal and state legislation guarantees a
person’s right to choose the home health care provider that he or she
wants in their home. This is an important right for health care consumers.
Hospital discharge plans must include an evaluation of a patient’s
possible need for medically necessary home health care. Hospitals are
required to provide a list of agencies that offer service in the patient’s
area of residence when home care needs are identified.
The Danbury VNA will provide services to
patients who are discharged from any hospital in Connecticut, New York,
New Jersey, Massachusetts, Rhode Island, Vermont, New Hampshire and Maine
or any hospital in the country.
